Our Christmas Self Referral Form can be filled in where the following is true:

• You are experiencing hardship and need support with providing Christmas gifts for your children
• At least one of your children is aged between 0-12 years
• You live in Cheshire West & Chester

You may include all family members permanently living with you at your address.

The closing date for referrals is 1st December.

Your referral will be prepared in November/December for delivery by 18th December

Once we have prepared your referral we contact you by phone to arrange delivery. Following your submission, please let us know any changes to your delivery address.
